It could be placed on specific pages.
Wufoo uses a likert scale.
In SurveyMonkey we need to upgrade to turn off the surveymonkey logo at the bottom. There is a pop up option. Also the SM product shows a good example of a sticky feedback button on the right hand side.
Clare thinks using the feedback block and embedding the survey in looks tidy. Better than the above 2 options.
